To reproduce:

1) Go to admin/structure/taxonomy
2) Click "add term" for one of your vocabularies
3) Make sure "auto alias" is checked; add the term.
4) Edit the term, by clicking "list terms" for the given vocabulary.
5) See that the checkbox for "auto alias" is no longer checked. *
6) Look up the alias that was generated. It's missing a taxonomy ID in the url, resulting in an alias to /taxonomy/term. *

Yes, my mistake. On a clean d7 + pathauto, it does work fine.

Sorry about that. I wonder what the issue is then...
