The first revision created (of a node, say) doesn't seem to get a revision UUID.

I can confirm this issue. This patch seems to fix the issue for me too. I'll leave it to the maintainer to mark as RTBC as I'm not sure if this is a bug or feature.

Strange, I haven't run into this. I guess this is in combination with alterations made by another module. I have however committed the patch since it doesn't seem to break anything in the tests.
