Extract location information from article content

If an incoming item does not already have location information assigned, check the start of the first line for a city (, country) location (using Geonames web services). If the article begins with a location (see below for examples) then add that city/country data and geonames-returned co-ordinates as the location of that node.

Could potentially also take advantage of MetaCarta's GeoTagger API.
