I just ran across this error while performing an update.php:
Fatal error: Call to undefined function: system_modules_util_form_alter() in /example.com/devSite/sites/all/modules/util/util.module on line 81
I had a fully functioning 5.1 install with lots of contrib modules, and was in the process of updating Advanced Poll, Javascript Tools, and Voting API, as per the ever-watchful Update_status module's harassment. :=)
Upon uploading these three newer modules, and then visiting update.php, I was greeted with the error. It's important to note that Util.module was working flawlessly before this. FYI.

senpai commentedI was greeted by this bug again on a different site with different modules. Disabling Utility module prior to running update.php is a viable workaround for now. It's just a hassle.
I wish I could figure out what's causing the error to trigger.

Same error. See it's been fixed in the dev branch. An official release after six months would be nice. Uninstalling for now, we'll play with dev later.
