Sometimes the page title isn't logged

This module has saved me a load of time as I was about to write something similar myself. I have noticed an intermittent bug - sometimes the page title isn't logged for a page. It isn't to do with specific nodes being viewed as the same node will sometimes show its title, and sometimes will just pick up the node/nn reference.
Has anyone else seen this behaviour?

I have never seen this behavior in my site. It would be nice if you give me a list of sample.

I've attached a screen shot that shows a copy of the log. The coloured boxes are log entries that refer to the same page. Some have the title, some have node/nn.
It's not too much of a problem, but I'm curious as to why it only does it *sometimes*!
