Submitted Paper becomes editable to the author after any decision

I have followed all the bugs and almost got it working completely, but discovered a very important bug.

1. After submitting the paper, it remains editable for the author until assigned to a reviewer.
2. The paper remains locked to the author for editing after the review and until a decision by the manager.
3. After a decision is made by the Manager, the submitter paper becomes editable again for the author ! This should become editable only if the decision is 'Modification Requested'

I have checked all permissions and am sure there are correct. Any advice or suggestions will be greatly appreciated. I have corrected quite a few small errors (spaces, brackets, etc) on the module files and once all is working will upload them to help all.
