But will also have an option to pay the appointments with a paypal similar (brazilian PagSeguro system)
Is it possible to have a clone of the Appointment Scheduling System Drupal based? I would like a flat rate for developing this, only setup and custom programming as I am a experienced Drupal themer

You may wish to try integrating the frontend of an existing booking system into Drupal. Have a look at Checkfront - there is a Drupal module here: http://drupal.org/project/checkfront.
