fail_mysqli() is a much more descriptive name than fail_mysql(), so I've fixed that. Anyone using fail_mysql in their own modules should just update their modules to use the new function name, backwards-compatibility isn't our responsibility, even if it is in the same major release.
The whitespace changes in the patch are just a bonus.

While I could commit this patch now that it's been marked RTBC, worst practice dictate that I should add any random participants in the issue queue as a co-maintainers, and have them commit their own patches.
