I received the following message:
I've downloaded and tried out your Outline module.
I think I've got it sort of working, but I'm not sure I've got my head
around it. In order to create an Outline page, I have to create the
content page then go back in and edit it to add the Outline features. Is
this correct? If so, how does a "default" content type come into play?
This points to a documentation problem, which needs to be fixed.
When you create a new volume, the default node type will set as the node type of the (first) node you put into that volume. But you can change it if you want the 'cover page' (or the top-level page) to be of a different node type of all the other pages that will be added into the volume later.
The default node type is set volume-wide (each volume having a default node type).
Also, a subsection of a volume can have a different node type (e.g. a sub-section can be made of 'pictures' instead of 'handbook pages'). In which case you don't use the default node type, but set the node type on the first node (the setting will apply to all the children).
Note that on the outline tab, you have the possibility to change the default node type for the whole volume. The new setting will be applicable to all the nodes within that module who have the child node type set as 'default' (i.e. not hard-set to any other node type).
