Unticking Generate automatic URL alias and creating custom alias is not being honored. It works the first time the node is saved. And it doesn't happen all of the time on a subsequent node save. It will revert back to the auto generated alias, and the tick box is ticked back on. It seems to happen at random times; especially on webforms.

I am 99% sure this is a duplicate of #936222: Merge in pathauto_persist module functionality to prevent losing manual aliases with node_save() calls. You can install http://drupal.org/project/pathauto_persist in the meantime to prevent this from happening.
