My cron is set to every hour, yet the message wasn't sent until 3:47pm on the "birthday". If there is a way to allow admin to choose a time of day, that would be cool.

Maybe integrating with the Job scheduler module could solve this, asking for support there: #1364926: User interface for managing jobs.

Yes, but not in a simple straight forward way. The first time the cron job was ever run after installing Birthdays goes into account as well. And the time might or might not shift, as days pass by. Really bad ;)
