Upload module won't work...

After MANY hours of reading on here and attempting to patch/tweak the upload module, I still cannot get the thing to work...

I am using drupal 5, upload.module 1.148

My permissions are set to 777, and no errors in drupal.

My steps:
1.log in
2.edit content
3.browse for file attachment
4.click attach
5.The progress bar appears, then after a second, goes right back to the upload (browse for file) form... no attachment table is shown, and no files are uploaded.
6.I check my folders via FTP, and no files are shown in either my 'files' or 'tmp' folders.
7.I have also installed the IMCE module to use with tinyMCE
8.in the IMCE, I also recieve no errors, however the same issue occurs... choose a file, click 'upload', file browser refreshes, but with no file in the list. I can, however, view the files that are currently in my 'files' folder, and I can also preview/add them and it all works great.

i have tried the patch available here: http://drupal.org/node/65153, however, no luck. I have also tried to switch up the favicon setting in the admin, but this clearly isn't that issue...

Any ideas? I've honestly spent about 15 hours on this in the last day & a half now with no luck...
