Make Revver associate IDs per user

Description
For a community website, we don't want just one affiliate / associate ID. I've listed this as Revver, but for any service where you can have a direct user link of some kind, it should probably be settable on a per user basis. Then the global setting is for the "default" ID to be used if people haven't filled it out.
...have I mentioned this is shaping up quite nicely? :P
