I know you are only really accepting minor maintenance on this module, but it would be nice to have an additional option to lock down the menu parent item per content type as well.
I've had two projects come up which needed the current functionality + the menu parent item locked down as well... This module for some reason was also quite hard to find... but now I have found this module I can go back and complete an old community project I started about a year ago...
Anyhow i've attached my modifications, let me know what you think!

I'll try to change it to better work UX-wise. With your patch I can select to force the menu parent and leave the menu force checkbox unselected.
I'll try to make it so that your checkbox is only available when the first checkbox is selected.

If you don't get time let me know and I may be able to finish this option off as part of the work i'm currently doing.
Would there be advantages to being able to lock the menu item but allowing choice over making adding it compulsory? Just playing the other side of the field...

Not sure if you would want to include force revisions in the menu module but it falls in with forcing content options... Maybe rename the module if you want to include this as well.

The rest of the issue (force menu parent item) is included in the next release. Thanks for posting!
