The standalone field_collection add/edit form (found at field-collection/field-name/add/node/123) shows the fields in an order that doesn't match anything at admin/structure/field-collections/field-name/fields or admin/structure/field-collections/field-name/display.

My apologies, when I realized this was actually Field Collection Table I found my way to http://drupal.org/node/1283310, which solves the issue (and which this issue is a duplicate of).
