When sending a custom message, it would be very handy if one could choose a design template to use, rather than have to manually add the necessary HTML to ensure custom messages have a consistent look to regular scheduled newsletters.

Setting to the 6.x-2.x-dev version, if this were added it would come after the 6.x-2.0 release. Also marking as minor, the custom message is a nice bell/whistle, and I don't want to overdesign a bell/whistle.

Moving into the 7.x branch, after I port this into D7, I'll investigate also allowing newsletters to be designed using Drupal's theming system in addition to using XML and XSL.
