Trim $file->filename to 255 characters

If an oEmbed title is longer than 255 characters, the MediaInternetOEmbedHandler class may create a file object which cannot be saved to {file_managed} on some database systems (MySQL auto-trims strings that are too long). We should make sure to truncate it to 255 characters first.
