If you log in as a different user on a computer which has already been used by someone who has posted a message to the shoutbox, then the username displayed in the box is the name of the previous user presumably stored in the cookie. Rather than the currently logged in user

Sorry forgot to day this only happens if you check the option "Default the name field to the logged in user name" and "Show the name field for logged in users"
