I've just released a patch to incorporate a feature that is in the imagefield.module where a default pathname can be set for a particular filefield CCK. I'm relying pretty heavily on the filefield.module on a project at work where we list multiple files for a product that are available for download and keeping a clean file structure for the files uploaded through the filefield is imperative.

I cannot release the development url of this site but the current site is http://www.c-sgroup.com which is being redone in Drupal, to be released sometime in June. Lots of custom themeing and contributed module implementations. I plan to post a full description of how the site was put together once it is released. If you're interested in certain aspects, I can post a few screenshots for you.

Looks like I didn't do my homework before working on this since it was already committed to HEAD with the additional feature of token support. Great stuff!
