When the Simplenews email format is set to HTML and tokens are used inside an anchor tag (e.g. the unsubscribe url token) then token replacement doesn't work. Tokens outside the anchor tag work fine and tokens in all contexts work when Pathologic is disabled.

Are the tokens not being replaced before the text hits the Pathologic filter? If not, that's definitely going to be a problem - if Pathologic can't see the "real" URLs, it won't know how to handle them correctly.
