Php notice: Undefined index: default_value_field [#1426242]

I am using an option element field with '#default_value_allowed' => FALSE, and getting the error mentioned in the title. Will attach a patch.

I'm getting ready to release Form Builder 1.0. So your fix came at a good time. I just rolled a 1.6 tag for Options Element containing only your fix and #1404988: Set default option keys to numbers and increment intellegently. It should be available in the next few minutes. :)
