When I enter the url admin/content/ad/configure/notify, it will result in the following error:

I see a blank screen, with the following error on the next page:

warning: call_user_func_array() [function.call-user-func-array]: First argument is expected to be a valid callback, 'ad_notify_settings_form' was given in /path/to/example.com/includes/form.inc on line 218.
