7.x Release [#1428804]

Since there hasn't been any activity on this module in quite awhile, I've attached a patch for a Drupal 7 version.

(Issue is tagged as 6.x because the field was required)

Hmm, sorry about that. Turns out not only do you need to browse for a file, it's also really important to click the attach button.

Also, I think I resolved #1068048: Filter not triggered when HTML contains attributes and possibly #584042: Better incasesensitive heading matching (not sure about the second one, code seems right, but it didn't work, but that could have been a result of other filters/the wysiwyg module).

There was still something wrong with the replacement loop.
Finally fixed that and the current code seems working here.
