This occurs in both the PDF table and PDF unformatted. I have added '$border = 1;' to all the PHP Code Before Output fields that I want to have borders. When a PDF template is not selected the borders showed up but when a template was selected they disappeared on the PDF. Opening this file in Adobe Illustrator revealed that the borders were actually there, just hidden by the PDF template image.

My first thought is a weight value or a output order. I so far have not located a place where this is being defined. Does anyone know how this template image is being inserted into the PDF file?

Update: saving the template PDF with a transparent background allows the cell border to be visible.
