Notice: Undefined variable: options in user_relationship_mailer_user() (line 205 of /mnt/apci/main/www/sites/all/modules/contrib/user_relationships/user_relationship_mailer/user_relationship_mailer.module).

$options doesn't exist so I just removed the ['#option'] considering 'checkbox' doesn't use it.

Here is a patch for the issue above. oops doesn't apply cleanly, wrong path. Jumped the gun. Adding another that does.
