Do I have to install it for every Site or can it be installed in sites/all/modules/
and robotstxt automatically handles the different sites?

Does robotstxt creates real robots.txt files or is the definition stored in
the database and provided in real time just by reqeust?

It installs anywhere where a module can install. It does not create actual files; it uses Drupal's menu system to handle requests for robots.txt.
