Double/Triple Content and CSS update

I have two problems:

1) When people submit a custom content type that I made, there is often 2 or 3 identical nodes created instead of just 1.

2) I updated my CSS file and uploaded it for UEI forum, but nothing has changed. I did put it in the correct place.
