Insert button disappears after one use, need to disable then re-enable Insert module to get it to work again. [#1460086]

Drupal 7.9. The Insert button shows up correctly, and works properly... once. After I save a page, if I go back to that same page or try uploading an image to any other page, the Insert button no longer appears.

I have to disable the module and then re-enable it to make it work again, for one time.

Any help would be appreciated.

I have no idea what could be causing this. I've put a lot of fixes in the the current Git code, so when 1.2 comes out I'd appreciate you giving that a shot. I personally haven't ever seen this problem, so I'll need instructions on reproducing it if it keeps happening after the next version.
