Themed output (upload.module-style file list)

Here's gordon's user interface improvement from #138748, split out from the AJAX uploading patch because it can stand on its own. It makes filefield's file list look like the one that upload.module provide, which looks a whole lot better than the current, slightly confusing user interface.

Try it out, it's great.
