Add support for account activation

This is the opposite of account expiration and I didn't think I'd need it when I started. With this turned on a person may register using a promotion code, but they won't be able to gain access to their account until the activation date is passed.

This needs configuration columns in the promotional code table, support in the cron hook, and a section on the settings page to configure it.
