Which order pane causes this?

Happens to me with this module installed. Problem stops with it disabled.

I can also confirm that #3 fixes the issue. However, this is not a good long-term fix because I need the Ubercart Discounts Alt module's functionality. Any ideas on how the issue can be permanently fixed?
