I have the default for an entire vocabulary set to DENY all modes except view, and on a per role basis have Add Tag. However, after rebuilding content permissions, I can still access TAC entities (with Revisions) as roles who should not have access.

Also when I edit an existing node and change the TAC term to another category, I don't get any permissions to edit as a lower level user without rebuilding content access permissions again.

I notice that in the node_access table, the taxonomy_access_role grants appear correct, however workflow_access grants still allow updates to the content- is this causing the conflict? How can I get the two to work together?

I should also state I am using Revisioning, TAC, and Workflow in conjunction.
