creating merged pdfs or bulk generate for thousands of forms

use case: we want to generate thousands of forms and have them all in one pdf - is there any way to do this? I see the page break function, just not sure that's what we want though, because we need to have a template page in the background -

it's a two page survey and we want to have the user's name and such on the survey and then have all of the survey's in one form rather than thousands of separate forms that must be printed separately.
