in the INSTALLATION part of the README.txt, you say :

When the module is enabled and the user has the "administer
actions" permission, an "actions" menu should appear under
Administer > Site configuration in the menu system.

but in fact, the menu doesn't appear under "Site configuration" but "Site building".

This version of Actions is not supported anymore. The issue is closed for this reason.
Please upgrade to a supported version and feel free to reopen the issue on the new version if applicable.
