I am adding files via the ftp copy method
and I keep getting a _0 attached to a file at random and there is no duplicate file in the folder sites/default/files, nore has there ever been a file uploaded with that name. any suggestions on how to trouble shoot this?

I have remove the file via edit / remove - saved then re - attached the file using the attach button and save as well as just attaching the file and choosing save with same results. I have this one file that I am working with right now and it is doing this 100% of the time..

If this has already been mention, I'm sorry did searching and I did not see this issue.

This can happen if your upload and attach directories are the same. See #1492374: Attach source reference existing file where it's the same as source.
