Same here. Additionally the BOOKMADESIMPLE configuration is no longer in admin/config and there is a link in the toolbar for BookMadeSimple settings which goes to blank page (white screen, viewing source provides nothing)

I can confirm this too...

On top of that, new nodes aren't added to the books with same settings as in 3.3 so i switched back.

I'm not sure what happened, but the last time I worked on a feature together with MarcElbichon the code looked like the one in the attachment. That version works great for me.

I don't know why but package 3.4 for Drupal 7 contains Drupal 6 version.
You can try dev version instead.
I 'll recreate a new version with correct files.
