Please include a check for file system access. I don't know how many minutes were wasted.
Here is what happens...everything is setup correctly except for write permissions to the file system. I visit the root on the new install and get the DB install screen. If I fill out the form and hit submit, and I get the same screen back completely blank. So as a user, I think, well, let's try that again. Wash, rinse, repeat, until it hits me, permissions!
If you wrote out a small file and tested for existence of that file - that's it. Do that right when you load the page.
