Taxonomy hierarchy doesn't get synced with menu hierarchy, because parent tid(ptid) doesn't ever get looked up.

Hello,
I have the same problem and I can reproduce by doing:
- Create a new menu
- Go to vocabulary edit
- tick rebuild menu on submit

The generated menu does not correspond to vocabulary hierarchy and some items are duplicated.

You can see that on http://uniqart.asia/new/ the left menu called "by industry" is generated wrong. It should be the same than in the exposed filter (select box).

I checked "do not display empty terms" so it is normal if all terms does not show but I have 1 product classified as "gifts boxes" inside the term "Wedding organizer" and the menu show 3 times "gifts boxes", 1 at the root level and 2 inside "Corporate"!?
