I've installed the module, and its operation for registered users was straightforward enough to figure out.

But, I'm not seeing a UI for anonymous/unregistered users to enter their email address when leaving a comment. There's the flag for "Notify me of follow-up comments posted here", but no field to enter an email address, let alone the other fields like name and math question shown in the example implementation of anonymous users leaving comments at marketingfan.com. How is the module supposed to know WHO to email when anonymous users leave comments?

Am using:
Latest version of comment_notify, just downloaded tonight
Drupal 4.7.6
Drupal access control had been set to allow anonymous users to post comments and post comments without approval

please check the settings for "Anonymous commenting" in the general comment settings

* Anonymous posters may not enter their contact information
* Anonymous posters may leave their contact information
* Anonymous posters must leave their contact information

and make sure you check the 2nd or 3rd option
