Clean up comment module

I tried but lost heart. I hope (not really, but I was wrong earlier) someone will run with this: beginning of a cleaning up of comment module. There is no point in posting a patch. I was rewriting from ground up picking up pieces as I was going. It's now outdated but can easily be brought up to date. Untested.
