I followed the config guide(s) but still I'm getting

Folder not found. Please refresh and try again.

When I try to upload an image via ckfinder. What causes this error? I have userfiles with required subfolders. Have been trying for hours ...

Please check status report page in your drupal admin panel. Check if there are any errorr reported by CKEditor module.
Please also check if you followed all steps in "Installing CKFinder" section of readme file.

Status report shows no errors. I followed the readme file over again twice. I have even uninstalled the module completely (including files and db config) and installed the "ckeditor for Drupal" zip from the main ckeditor.com, which appears to have most of the readme instructions already in place.

I have set up ckfinder just fine on a handful of other sites. But this site runs on Microsoft IIS 7. Maybe that is it?

Do you see any errors in Firebug or similar tool ?
Can you check if there are errors in server logs ?
