Date Field Problem / Locating for links

Hi

I am currently have having an issue with two sections of the form builder module (New to drupal and this is my first time of using this module)

Item1:
I have added a date field to one of my forms but it is currently giving this error:

* warning: checkdate() expects parameter 1 to be long, string given in /var/www/vhosts/officenetworkplace.co.uk/subdomains/dev/httpdocs/includes/form.inc on line 1218.
* The specified date is invalid.

this is when i set the date to dec this year, the date also apears to default to 1900.

Item2:
I have looked through the site for how to link the forms into the menu system - or more specifically - how to create a link to a form to no avail so far.

Any hints would be appreciated - also there is a line in the release notes that states - this build is full of bugs do not use, it might be worth removing that line.

I am using the latest version of drupal (5.x) and it is installed on a linux server with php4 and mysql4 - everything else is working fine.
