Improve what content of the ToS node is shown during checkout

I'd like to do the following changes to the way the ToS node is handled during checkout:

* allow the admin to select the node display (teaser, full, other)
* allow the admin to hide the node title, since it is often redundant to the fieldset.

Since it is a (relatively!) big patch, sending it here for review first.
