As I'm testing a lot for migrating a custom cms with my custom .csv this would be a nice feature.
For now need I need to empty the table "file_managed" prior to a new import, otherwise I'll get a
duplicate-entry error.

this possibly relates to the issues?

I think it is very likely that it is related to that issue. Bug in the core file module it seems. Luckily the issue has some patches, have you tried them?
Unfortunately I can not try it myself at the moment as I don't get to see any user pictures at all. Let alone have trouble with duplicates :-)
