The Title, Middle Name(s), Family and Generational text boxes are missing from the add Household contact entry page. The labels are displayed but the fields are not.

Since the Family field is required, the form results in an error.

Tested with:
CRM Core 7.x-1.x-dev (2012-May-02)
Name 7.x-1.5+1-dev (2012-May-10)

I don't have this error with Name 7.x-1.5 module, but there is something we need to do with labels.
