Data not being added to database correctly

I worked on issue http://drupal.org/node/1594140, Database error when filling out a poll as anonymous, and was able to get to the point where anon can vote when limited by IP.

However, the data is not going into the database correctly. The data for field_table and field_name are being entred as:

field_revision_field_poll field_poll

(see screenshot)

I haven't had a chance to try to work this out...
