Ho does spam work exactly when it requires email confirmation? Our sites are getting spammed like crazy, and I have the spam module installed.

Are you using the Captcha and textimage modules ? They can be helpful.

One of my Drupal sites was getting about twenty spam profile submissions a day. I installed MyCaptcha and added it to the profile sign up form. Haven't had a spam profile submission since.

Yep!. In my site, this solved the problem: Captchas + Akismet (strong config) and check the registry of users (delete fake users).
