I've had a similar problem with both the 4.7 and 5.0 version of this module.

I never have a problem uploading the file and it appearing in the list of attached files prior to submitting and article, but then very often it disappears after submitting. After 2 or 3 attempts, it may finally attach the file.

Could this be a problem with the module or clashing with another module? Or could it be something to do with my host's settings (MediaTemple)?

I am also having this problem with 5.2 version. I recently upgraded to 5.2 and moved my site to a new server at the same time. Don't know if it's the new server or the new version (was on drupal 5.1 previously). The permissions on my file system are 777. Any ideas?
