Issue Summary
With text analysis selected, when Mollom is unsure if my comment is spam or not I get the word verification message, but the CAPTCHA form does not display. If I have the protection mode set to just CAPTCHA, the CAPTCHA form displays and works fine.
It seems it is correctly recognising a comment that requires a CAPTCHA fallback but not displaying the CAPTCHA form.
My settings are:
Protection mode: Text analysis
Text analysis checks: Spam + Profanity
Text fields to analyse: Title + Comment
Text analysis accuracy: Strict
When text analysis identifies spam: Automatically discard the post
The site is in German.

Hm. This is known to work and actually covered by tests.
I suspect that some other module that is installed on your site conflicts with Mollom. Do you have any other module installed that might affect the placement or interaction or "whatever" of the comment form? :)
