Exposed filters completely broken (using Better Exposed Filters)

Setup:
* Insert view 7.x-2.0
* Better Exposed Filters 7.x-3.x-dev
* Views 7.x-3.3 (no patches)
* Core 7.14

I've got a view using several fields for exposed filters: a text select list, a date (using core updated date with BEF's jquery date picker), and the title. Previewing the view shows a perfectly fine set of exposed filters but when rendered using insert view in a node body, I'm getting incomplete markup and no filters at all (see broken-exposed-filters-html.txt)

I've tried turning the exposed filter type back to basic but nothing changes (yes I cleared cache etc), so it may not be a fault of BEF. I will try to look into this myself but would appreciate some help.

Edit: worth noting that the filters render fine normally in panels etc.
